# Record a visit

> From choosing the patient to a finished note, in the web app.

  <Step title="Choose the patient">
    Sidebar → **AI Scribe** → **Record**. Type in **"Search or add patient name"**. Pick an existing patient or choose **Add "…" (New patient)**.

    If your EHR is connected, a **From EHR** group appears so you can pick the chart directly. Some EHRs get their own **Search in …** box on this screen.

    <Info>You cannot start recording without a patient selected. That is what lets HealOS pull prior-visit context.</Info>

  <Step title="Confirm consent">
    The first thing on the session screen is **Patient Consent for Recording**. Tell the patient the visit is being recorded, then click **I Confirm Patient Consent**.

    You can turn this prompt off under Account Settings → **Show Consent Prompt**.

  <Step title="Pick the template and note style">
    * **Choose template**: start with **New Patient Visit** or **Follow-Up Patient Visit**. Custom templates appear here once you create them.
    * **Select note quality**: **Balanced**, **Fast**, or **Detailed**. Detailed takes longer but produces the most complete note.

    Optional but useful:

    * **Patient context**: a free-text box for anything the AI should know. Read-only once recording starts.
    * **Patient Documents**: upload reports, prior notes, or intake forms. The note can pull from them.

  <Step title="Record">
    Click **Start recording**. Allow microphone access when the browser asks. **Pause** and **Resume** as needed. Click **End Session** when the visit is over.

    <Warning>Record for at least **1 minute**. Stopping earlier shows a **Recording Too Short** warning because very short audio produces thin notes.</Warning>

## Recording modes

Use the small arrow next to **Start recording** to pick a microphone and a **Recording Mode**.

    Audio uploads in 30-second pieces while you talk. The transcript appears after you end the session. Works well on ordinary office Wi-Fi.

  <Tab title="Live">
    The transcript appears on screen as you speak. Useful when you want to glance at what was captured mid-visit. Needs a stable connection.
  </Tab>

  <Tab title="Offline">
    Saves locally and uploads when you are back online. Use this on unreliable Wi-Fi or when travelling between sites.
  </Tab>

<Tip>
  Keep the device within a few feet of the conversation. Almost every "no note was generated" report turns out to be audio with no audible speech: laptop across the room, another app holding the microphone, or a muted input.

## Other ways to create a note

Below the recorder, under **"or use a different input"**:

    Write the encounter as text and click **Generate Note**.

## What happens next

After **End Session**, HealOS finalizes the transcript and generates the note. The session page shows a **Note** tab, a **Transcript** tab, and a **Session Summary** card.

* If generation takes more than a minute you will see **"Taking too long? Try again"**.
* If it fails outright you will see **Note generation failed** with a **Regenerate Note** button, which rebuilds from the saved audio.
